Outreach & Engagement

Outreach & Engagement provides targeted outreach to employees with identified health and productivity risks to offer program supports and engage them in taking action for behavior change. Because individuals who could benefit most from the services may be the least likely to call with a self-referral model, adding Outreach and Engagement ensures that the individuals who would most benefit from the program are offered program resources.

 

Identification

Individuals are identified as at-risk for health and productivity issues via self-assessment, self- or cross-referral, employee assistance programs (EAP), disease management programs, or other benefits services.

 

Proactive Outreach

Following carefully established guidelines outreach calls are made to identified at-risk employees. With the first call the employee is introduced to the program, and learns the benefits of participation including any associated incentives you choose to provide. Employees are encouraged to participate but may elect to “opt out” of participation.

 

Assessment and Action Plan

Our experts assess readiness to make behavior changes, describe all programs the employee is eligible for, and can process program enrollment right on the phone or do a warm transfer directly to the service the employee needs. They perform a comprehensive assessment that broadens the scope of risk identification to uncover concomitant issues that may be impacting the individual’s health or exist as barriers to making health improvements.

 

Following the assessment, the coach will guide the employee through an exploration of options. The coach will help the individual set goals and create a realistic plan of action.

 

Referral

As needed, cross-referral integrates your company’s other benefits programs such as EAP, work-life or other company provided programs in order to improve health and productivity. Health coaching for the initial risk identified issue follows the needs and stage of readiness of the participant.

 

Follow-up

We follow-up with employees after six months to check on program engagement and wellness plan progress.
